The patient with aortic stenosis may have reduced left ventricular outflow with a relatively normal valvular architecture and little or no myocardial hypertrophy. For such a patient, surgery directly on the outflow structures may involve serious problems and "trade-offs". Construction of a supplementary conduit from the ventricular apex to the abdominal aorta can provide a valid alternative to more conventional approaches.
