Five years ago the terms 'purchasing' and 'providing' in the NHS were uncomfortable if not unfamiliar terms. But with the internal market in health care now well established, new developments in research and development and in education and training are introducing similar market principles and disciplines. Over the next 5 years managers and clinicians in trusts and health authorities will need to consider the links between these three market systems and how they can be harnessed to improve patient care.
