Perilunate dislocation is an uncommon injury of the wrist due to hyperextension. The factors affecting its prognosis are still controversial. The aim of the present study was to review its fracture types, associated injuries, timing of surgery, different fixation methods and prognostic factors.
Fourteen patients with perilunate dislocation or fracture dislocation were treated by open reduction and internal fixation, and were followed for at least 12 months. A functional scoring system and X-ray findings were used for follow-up evaluation.
Two patients had excellent results, six patients had good results, and four had fair results. Only two patients had poor results. Treatment delayed over one month, transscaphoid perilunar fracture dislocation, or fixation of the transscaphoid fracture with Kirschner wire was associated with poorer results.
Early diagnosis of perilunate dislocation, prompt open reduction and rigid fixation for fracture combined with ligament repair can give more promising results.
月骨周围脱位是一种因过度伸展导致的腕部少见损伤。影响其预后的因素仍存在争议。本研究的目的是回顾其骨折类型、合并损伤、手术时机、不同固定方法及预后因素。
14例月骨周围脱位或骨折脱位患者接受切开复位内固定治疗,并随访至少12个月。采用功能评分系统和X线检查结果进行随访评估。
2例效果极佳,6例效果良好,4例效果尚可。仅2例效果较差。治疗延迟超过1个月、经舟骨月骨周围骨折脱位或用克氏针固定经舟骨骨折与较差的结果相关。
月骨周围脱位的早期诊断、及时的切开复位、骨折的坚强固定及韧带修复可取得更理想的结果。
