A case of 18 years old female treated for 11 cm femoral deficit due to comminuted fracture and secondary osteomyelitis is described. Free vascularized double barrel fibular graft has been used. The detailed delineation of surgical technique is given. Ten months after surgery complete healing and incorporation of the graft was found; the external fixation was removed. Excellent functional result has been achieved.
