Surgical reduction has been proposed to reduce left ventricular wall stress and improve geometry in patients with left ventricular failure. We describe 2 cases of direct surgical reduction in ischemic cardiomyopathy. Although left ventricular function improved in both, late ventricular dysrhythmia negated the result.
