Anatomical anomalies are recognised but rare causes of acute pancreatitis. We report a case or a 33-year-old woman with recurrent attacks of acute pancreatitis associated with a ampullar intestinal duplication.
This patient was clinically studied, and a barium contrast study of the duodenum showed a filling defect in the second part of the duodenum, that was thought to be a periampullary lipoma. Endoscopic retrograde cholangiopancreatography was scheduled, but cannulation of the papillary orifice failed.
Laparotomy and surgical resection of the lesion were performed. Pathological study of the specimen revealed a duodenal duplication filled with multiple calculi.
This entity should be considered among the lesions that can cause recurrent acute pancreatitis.
解剖学异常是公认的但却是急性胰腺炎的罕见病因。我们报告一例33岁女性,其反复发生急性胰腺炎发作,与壶腹周围肠重复畸形有关。
对该患者进行了临床研究,十二指肠钡剂造影显示十二指肠第二部有充盈缺损,当时认为是壶腹周围脂肪瘤。计划进行内镜逆行胰胆管造影,但乳头开口插管失败。
进行了剖腹手术并切除病变。标本的病理研究显示十二指肠重复畸形内充满多个结石。
在可导致复发性急性胰腺炎的病变中应考虑到这种情况。
