Traumatic pneumocephalus results from severe penetrating craniocerebral injury. Resorption of air depends on its location in the cranial cavity, concurrent injury to the brain. In the subarachnoid space resorption of a small amount of air (less then 60 cm3) occurs within the shortest time possible. In substantia medullaris air is not resorbable. In all cases cranium is supposed to be air-tight in order that full resorption might take place. This will determine the choice of the treatment option to be adopted.
