A case of an unusual association between hernia and testicular infarction, namely torsion of a normally descended testis within a persistent processus vaginalis, is presented. The advantages of the inguinal ingulinal approach for the exploration of suspected torsion are discussed. Although an operation was performed more than 30 hours after the onset of symptoms the testicle was viable and recovered.
