Dislocation of the posterior tibial tendon has been reported rarely in the English literature. We report a case of traumatic dislocation of the posterior tibial tendon. The diagnosis was delayed for months and was ultimately made by magnetic resonance imaging, which accurately determined the location of the tendon. At the time of surgical exploration, no predisposing anatomic abnormalities were noted. A new pseudoretinaculum had developed that prevented recurrent subluxation or dislocation. Surgical relocation of the tendon and repair of the retinaculum resulted in a good outcome.
