Impaired thyroid function-hypothyroidism, is associated with changes in the spectrum of serum lipids. Thyroid hormones increase the number and activity of LDL receptors in the liver and peripheral tissues and have also a direct impact on the triacylglycerol synthesis in the liver. If their amount in the organism is inadequate, the metabolic turnover of LDL-cholesterol and triacylglycerols is reduced and subsequently a rise of the LDL-cholesterol and triacylglycerol plasma concentration occurs. The patients are then frequently referred with the diagnosis of hyperlipoproteinaemia to a lipid clinic for treatment, the hypothyroidism remaining undetected. In the submitted study the authors had the objective to assess how many patients referred to lipid clinics on account of a high cholesterol level suffered from undetected and thus untreated hypothyroidism. Ex post it was decided to examine the thyroid function in all patients attending the lipid clinic, IPZV Zlín, incl. those who are on the records and had treatment for several years. It proved thus possible to assemble a relatively large group of patients.
甲状腺功能受损——甲状腺功能减退,与血清脂质谱的变化有关。甲状腺激素可增加肝脏和外周组织中低密度脂蛋白(LDL)受体的数量和活性,并且对肝脏中的三酰甘油合成也有直接影响。如果机体中甲状腺激素的量不足,LDL胆固醇和三酰甘油的代谢周转就会降低,随后LDL胆固醇和三酰甘油的血浆浓度就会升高。这些患者随后常常因高脂蛋白血症的诊断而被转诊至脂质门诊进行治疗,而甲状腺功能减退仍未被发现。在本提交的研究中,作者的目的是评估有多少因高胆固醇水平而转诊至脂质门诊的患者患有未被发现因而未得到治疗的甲状腺功能减退。事后决定对所有前往兹林市IPZV脂质门诊就诊的患者,包括那些有记录且已接受数年治疗的患者,进行甲状腺功能检查。结果证明这样有可能召集到相对大量的患者。
