A 45-year old black man with a carotid-ophthalmic artery aneurysm developed sudden loss of vision in his left eye, a mid-dilated pupil, and pain on movement of the left eye. A temporal island of vision was present in the left eye and the right visual field was normal. There were no other neurologic abnormalities. The aneurysm spontaneously ruptured, resulting in the death of the patient.
