The authors discuss the present approach to acute cholecystitis and the difficulties encountered in videolaparoscopic treatment. The effective usefulness of this new approach needs to be very carefully evaluated and the surgeon must bear in mind the patient's safety and the surgical risk. Therefore, in cases of very serious difficulties or increased risk, it is mandatory to convert the procedure to an open cholecystectomy.
