Abstract

OBJECTIVE

To investigate the sensitivity of distortion product otoacoustic emissions (DPOAE) and auditory brain-stem response (ABR) in neonatal hearing screening.

METHODS

DPOAE and ABR were conducted on 99 neonates (198 ears) at risk of hearing impairment as initial screening tests. One month later rescreening tests were conducted: DPOAE was conducted again on those ears failing this test during the initial screening test and ABR was conducted again on the ears failing this test during the initial screening test Those ears failing the DPOAE and ABR tests during the rescreening tests were examined with electric otoscope, diagnostic ABR, etc. to get confirmed diagnosis.

RESULTS

54 ears (27%) failed the DPOAE test during the initial screening and 14 out of the 54 ears (14/198, 7%) still failed the DPOAE test during the rescreening. 26 ears (12%) failed the ABR test during the initial screening and 9 out of the 16 ears (9/198, 4.5%) still failed this test during the rescreening. Two babies were confirmed as with severe sensorineural hearing loss, one unilateral and one bilateral. The sensitivity was 97% for DPOAE and 99% for ABR.

CONCLUSION

ABR is a little bit more sensitive then DPOAE in neonatal hearing screening. Two-stage testing procedure increases the sensitivity of neonatal hearing screening.

目的

探讨畸变产物耳声发射(DPOAE)和听性脑干反应(ABR)在新生儿听力筛查中的敏感性。

方法

对99例有听力障碍风险的新生儿(198耳)进行DPOAE和ABR检查作为初筛。1个月后进行复筛:对初筛时DPOAE检查未通过的耳再次进行DPOAE检查,对初筛时ABR检查未通过的耳再次进行ABR检查。对复筛时DPOAE和ABR检查未通过的耳,用电耳镜、诊断性ABR等进行检查以确诊。

结果

初筛时54耳(27%)DPOAE检查未通过,其中14耳(14/198,7%)在复筛时DPOAE检查仍未通过。初筛时26耳(12%)ABR检查未通过,其中9耳(9/198,4.5%)在复筛时该检查仍未通过。2例婴儿被确诊为重度感音神经性听力损失,1例单侧,1例双侧。DPOAE的敏感性为97%,ABR的敏感性为99%。

结论

在新生儿听力筛查中,ABR的敏感性略高于DPOAE。两阶段检测程序可提高新生儿听力筛查的敏感性。
