The value of various radiological examinations was analysed with reference to 75 renal transplants which had developed complications. If the renal poles are marked with metallic clips, the size of the transplant can be estimated by abdominal x-rays. Abnormal size was found exclusively in severe rejection reactions, but only about 50% of acute rejections showed such a change. Contrast examinations were found to be valuable in the diagnosis or exclusion of anatomically localised lesions such as obstructions or anastomotic leaks. They were unsatisfactory in the diagnosis and differential diagnosis of diffuse parenchymatous abnormalities such as rejection or damage during storage.
