Abstract

During the second half of the 20th century, due to anti-tuberculosis activities including BCG vaccination, the number of patients affected by tuberculosis was constantly declining in Serbia and in the majority of European countries. Nowadays Serbia and Montenegro (S&M) is a country with a moderate incidence of TB stable during the last 10 years and with potential risk of worsening of the epidemiological situation. The increase of newly diagnosed cases in 2002 has already been notified. The coverage of newborns with the BCG vaccination, still mandatory in S&M, is 96% on average, and depends on the region. Revaccination at the age of 10 years was discontinued in 2002. The aim of this paper was to review the current status of BCG vaccination in different countries, data on it's efficacy, duration of protection against tuberculosis, cost-effectiveness studies and general plans on vaccination compared to the WHO recommendations and national legislation. Although some of the International Union Against Tuberculosis and Lung Diseases (IUATLD) criteria for discontinuation of BCG vaccination have been fulfilled, current TB epidemiological situation in S&M requires continuation of BCG vaccination at birth.

在20世纪下半叶,由于包括卡介苗接种在内的抗结核活动,塞尔维亚和大多数欧洲国家的结核病患者数量持续下降。如今,塞尔维亚和黑山(塞黑)是一个结核病发病率中等的国家,在过去10年中保持稳定,但存在流行病学形势恶化的潜在风险。2002年新诊断病例的增加已经上报。在塞黑,卡介苗接种对新生儿仍然是强制性的,平均接种覆盖率为96%,且因地区而异。2002年停止了10岁时的复种。本文的目的是回顾不同国家卡介苗接种的现状、其有效性数据、预防结核病的保护期限、成本效益研究以及与世界卫生组织建议和国家立法相比的总体疫苗接种计划。尽管国际防痨和肺部疾病联盟(IUATLD)关于停止卡介苗接种的一些标准已经达成,但塞黑目前的结核病流行病学形势要求继续在出生时接种卡介苗。
