To investigate the levels of serum lipid at 36 - 42 weeks' gestation and their relationship to newborn weight in pregnant women with gestational diabetes mellitus (GDM) and type 1 and type 2 diabetes mellitus (DM).
This study included 30 normal pregnant women, 40 patients with GDM, 30 patients with type 1 DM and 30 with type 2 DM. Subjects with complications including hypertension or thyreogenic diseases were excluded. Fasting serum triglyceride, total cholesterol, high density lipoprotein cholesterol and low density lipoprotein cholesterol were measured by enzymatic method.
(1) Serum triglyceride levels in pregnant women with GDM, type 1 DM, type 2 DM and normal controls were (4.0 +/- 1.4) mmol/L, (2.9 +/- 0.8) mmol/L, (4.1 +/- 2.5) mmol/L and (2.7 +/- 0.9) mmol/L respectively; the total cholesterol levels were (6.5 +/- 3.0) mmol/L, (6.2 +/- 2.8) mmol/L, (6.4 +/- 3.2) mmol/L and (6.0 +/- 3.0) mmol/L respectively; low density lipoprotein cholesterol levels were (3.2 +/- 1.3) mmol/L, (3.2 +/- 1.3) mmol/L, (3.3 +/- 1.1) mmol/L and (3.2 +/- 1.0) mmol/L respectively; high density lipoprotein cholesterol levels were (1.6 +/- 0.5) mmol/L, (1.4 +/- 0.5) mmol/L, (1.5 +/- 0.4) mmol/L and (1.6 +/- 0.4) mmol/L respectively; the newborn weights were (4108 +/- 544) g, (3323 +/- 457) g, (4111 +/- 263) g, and (3463 +/- 516) g respectively. Triglyceride levels in pregnant women with GDM and type 2 DM were significantly higher than in normal controls and type 1 DM patients (P < 0.01). (2) Triglyceride levels correlated with birth weights independently (r = 0.39, P < 0.01).
Triglyceride levels at 36-42 weeks' gestation were elevated in pregnant women with GDM and type 2 DM, suggesting that they might have more obvious insulin resistance. In pregnant women with DM, a positive and significant correlation was found between triglyceride levels and newborn weights.
探讨妊娠36 - 42周时,妊娠期糖尿病(GDM)及1型和2型糖尿病(DM)孕妇的血脂水平及其与新生儿体重的关系。
本研究纳入30例正常孕妇、40例GDM患者、30例1型DM患者和30例2型DM患者。排除有高血压或甲状腺疾病等并发症的受试者。采用酶法测定空腹血清甘油三酯、总胆固醇、高密度脂蛋白胆固醇和低密度脂蛋白胆固醇。
(1)GDM孕妇、1型DM孕妇、2型DM孕妇和正常对照组的血清甘油三酯水平分别为(4.0±1.4)mmol/L、(2.9±0.8)mmol/L、(4.1±2.5)mmol/L和(2.7±0.9)mmol/L;总胆固醇水平分别为(6.5±3.0)mmol/L、(6.2±2.8)mmol/L、(6.4±3.2)mmol/L和(6.0±3.0)mmol/L;低密度脂蛋白胆固醇水平分别为(3.2±1.3)mmol/L、(3.2±1.3)mmol/L、(3.3±1.1)mmol/L和(3.2±1.0)mmol/L;高密度脂蛋白胆固醇水平分别为(1.6±0.5)mmol/L、(1.4±0.5)mmol/L、(l.5±0.4)mmol/L和(1.6±0.4)mmol/L;新生儿体重分别为(4108±544)g、(3323±457)g、(4111±263)g和(3463±516)g。GDM孕妇和2型DM孕妇的甘油三酯水平显著高于正常对照组和1型DM患者(P<0.01)。(2)甘油三酯水平与出生体重独立相关(r = 0.39, P<0.01)。
GDM孕妇和2型DM孕妇在妊娠36 - 42周时甘油三酯水平升高,提示其可能存在更明显的胰岛素抵抗。在糖尿病孕妇中,甘油三酯水平与新生儿体重呈显著正相关。
