Three cases of Henoch-Schönlein purpura (HSP) with elevated plasma D-dimer levels are described. In one case of acute course HSP we observed the beneficial influence of plasma and nadroparine treatment on the course of disease. In two cases plasma D-dimer increase did not correlate with severity of the disease. The possibility of coagulation disturbances in HSP with D-dimer rise (as well as decrease of plasma factor XIII and platelets described in relevant literature) should be taken into consideration. These results suggest presence of mild intravascular clotting process in course of the disease. However, we were not able to clarify the pathogenesis of the high D-dimer serum concentration in HSP. Thus, it is suggested that further research on the discussed condition should be conducted.
本文描述了3例血浆D-二聚体水平升高的过敏性紫癜(HSP)患者。在1例急性病程的HSP患者中,我们观察到血浆和那屈肝素治疗对疾病进程有有益影响。在另外2例患者中,血浆D-二聚体升高与疾病严重程度无关。应考虑HSP患者出现D-二聚体升高时发生凝血紊乱的可能性(以及相关文献中描述的血浆因子XIII和血小板减少)。这些结果提示在疾病过程中存在轻度血管内凝血过程。然而,我们未能阐明HSP患者血清D-二聚体浓度升高的发病机制。因此,建议对该疾病进行进一步研究。
