Abstract

OBJECTIVE

To investigate the relationship between cerebral vasospasm and occurrence of delayed ischemic neurological deficit (DIND).

METHODS

The clinical records and radiographic images of 118 patients with subarachnoid hemorrhage admitted during last 5 years were reviewed. The incidence,degree and localization of cerebral vasospasm were evaluated, and morbidity of related DIND was analyzed. Patients with cerebral vasospasm were divided into three groups: Group MCA (middle cerebral artery), Group ACA (anterior cerebral artery) and Group ICA (intracranial carotid artery) according to the location of cerebral vasospasm. The consistency of DIND and image of cerebral infarction were examined.

RESULT

There was a weak correlation between cerebral vasospasm and incidence of DIND (r=0.22; P=0.016). The incidence of DIND was increased with severity of cerebral vasospasm (U=2.589, P<0.05). The group MCA had a significantly higher incidence of DIND than that of ACA and ICA groups (68.0% compared with 36.7% and 25.0%, respectively, chi(2)=8.195, P=0.004), the difference between later two groups was not statistically significant (chi(2)=0.646, P=0.421).

CONCLUSION

Cerebral vasospasm may be an important factor leading to DIND occurrence; the severity and location of cerebral vasospasm is related to the incidence of DIND.

目的

探讨脑血管痉挛与迟发性缺血性神经功能缺损(DIND)发生之间的关系。

方法

回顾性分析过去5年收治的118例蛛网膜下腔出血患者的临床记录和影像学资料。评估脑血管痉挛的发生率、程度及部位,并分析相关DIND的发病率。根据脑血管痉挛的部位,将脑血管痉挛患者分为三组:大脑中动脉组(MCA组)、大脑前动脉组(ACA组)和颈内动脉组(ICA组)。检查DIND与脑梗死影像的一致性。

结果

脑血管痉挛与DIND发生率之间存在弱相关性(r = 0.22;P = 0.016)。DIND的发病率随脑血管痉挛严重程度的增加而升高(U = 2.589,P < 0.05)。MCA组DIND的发病率显著高于ACA组和ICA组(分别为68.0%、36.7%和25.0%,χ² = 8.195,P = 0.004),后两组之间的差异无统计学意义(χ² = 0.646,P = 0.421)。

结论

脑血管痉挛可能是导致DIND发生的重要因素;脑血管痉挛的严重程度和部位与DIND的发病率有关。
