Five cases of palmaris longus muscle in the carpal tunnel responsible for median nerve compression are reported. This represents less than 0.5% of all carpal tunnel syndromes operated over a ten year period. A review of the literature reports this abnormality in about 0.5% of tendons examined. It is important to recognise these forms for carpal tunnel and tendon transfer surgery.
