To evaluate the impact of childhood trauma on psychopathology in 57 patients with first-episode schizophrenia.
Psychopathology was assessed by Brief Psychiatric Research Scale (BPRS), Scale for the Assessment of Positive Symptoms (SAPS) and Scale for the Assessment of Negative Symptoms (SANS) at first admission. Childhood trauma was assessed by Childhood Abuse Questionnaire and Childhood Trauma Questionnaire (CTQ) after discharge.
Frequencies of childhood sexual abuse (CSA), emotional abuse (CEA) and physical abuse (CPA) were reported by 29.8%, 40.9% and 13.6% of the patients respectively. Histories of childhood emotional neglect (CEN) and physical neglect were found in 29.5% and 20.5% of the patients respectively. The patients reporting CSA had higher SAPS scores at admission, and had more suicide attempts before admission. The patients with history of CEA had more hallucinations and delusions of mind reading at admission. CPA, CEA and mean scores of CTQ correlated with the number of siblings.
Our findings suggest that childhood trauma may alter the presentation of schizophrenia in first admission.
评估童年创伤对57例首发精神分裂症患者精神病理学的影响。
首次入院时,采用简明精神病学研究量表(BPRS)、阳性症状评定量表(SAPS)和阴性症状评定量表(SANS)评估精神病理学。出院后,通过儿童期虐待问卷和儿童期创伤问卷(CTQ)评估童年创伤。
分别有29.8%、40.9%和13.6%的患者报告有童年性虐待(CSA)、情感虐待(CEA)和身体虐待(CPA)史。分别有29.5%和20.5%的患者有童年情感忽视(CEN)和身体忽视史。报告有CSA的患者入院时SAPS评分较高,入院前有更多自杀未遂行为。有CEA史的患者入院时有更多幻觉和读心妄想。CPA、CEA和CTQ平均得分与兄弟姐妹数量相关。
我们的研究结果表明,童年创伤可能会改变首发精神分裂症患者首次入院时的临床表现。
