The objective of this study was to investigate the association between several medical interview behaviors and patient satisfaction.
The subjects were 158 new patients who visited an outpatient facility of a university hospital in Japan. All medical interviews were videotaped and reviewed by a trained rater using a medical interview rating scale (Takemura Medical Interview Rating Scale) for evaluating medical interview behaviors. To measure patient satisfaction, a self-administered questionnaire was also developed. Both the rating scale and the questionnaire were assessed for validity and reliability beforehand.
A significant positive association was found between the behaviors of reflection and legitimation on the one hand, and patient satisfaction on the other. The positive association between reflection and patient satisfaction existed after adjusting for both the duration of the interview and the other medical interview behaviors used. The association between legitimation and patient satisfaction also existed after adjusting for the duration of the medical interview but disappeared after adjusting for the other medical interview behaviors used. When we investigated the strength of the relationship between each medical interview behavior and patient satisfaction, reflection was found to be the strongest determinant of patient satisfaction.
This research revealed a significant positive association between reflection or legitimation and patient satisfaction in an actual clinical practice setting.
本研究的目的是调查几种医学问诊行为与患者满意度之间的关联。
研究对象为158名前往日本一家大学医院门诊就诊的新患者。所有医学问诊均进行录像,由一名经过培训的评估者使用医学问诊评定量表(武村医学问诊评定量表)对医学问诊行为进行评估。为了测量患者满意度,还编制了一份自填式问卷。评定量表和问卷均预先进行了效度和信度评估。
一方面,反思行为和合法化行为与另一方面的患者满意度之间存在显著的正相关。在对问诊时长和其他使用的医学问诊行为进行调整后,反思与患者满意度之间的正相关依然存在。在对医学问诊时长进行调整后,合法化与患者满意度之间的关联也存在,但在对其他使用的医学问诊行为进行调整后消失。当我们调查每种医学问诊行为与患者满意度之间关系的强度时,发现反思是患者满意度的最强决定因素。
本研究揭示了在实际临床实践环境中,反思或合法化与患者满意度之间存在显著的正相关。
