Isolated tuberculosis (TB) of posterior elements is extremely rare. Only few cases are reported in literature but none described isolated spinous process involvement in the absence of disease process in any other part of the body.
The authors report a case of isolated tuberculous involvement of C2 spinous process, which to the best of our knowledge, is first such case report.
A case report of isolated C2 spinous process TB.
Sixteen-year-old girl presented with two-month history of slow growing, painless lump at the back of her neck without any other symptoms. X-ray showed erosion of C2 spinous process. MRI also suggested an abscess.
Patient underwent incision and drainage of abscess and debridement of involved bone. Cultures and histopathology confirmed TB. Patient responded to antituberculous therapy.
Posterior element TB is a rare condition but in countries where TB is common, its possibility should always be considered. Isolated TB of parts of posterior elements can occur in absence of disease elsewhere.
孤立性椎体后部结核极为罕见。文献中仅有少数病例报道,但均未描述在身体其他部位无病变的情况下孤立性棘突受累的情况。

一名16岁女孩,颈部后方出现缓慢生长、无痛性肿块2个月,无其他症状。X线显示C2棘突侵蚀。MRI也提示有脓肿形成。
患者接受了脓肿切开引流及受累骨质清创术。培养及组织病理学检查确诊为结核。患者对抗结核治疗有反应。
椎体后部结核是一种罕见疾病,但在结核病常见的国家,应始终考虑到其可能性。椎体后部部分的孤立性结核可在身体其他部位无病变的情况下发生。
