To evaluate the late development of open angle glaucoma (OAG) after vitrectomy and to compare the rate of postvitrectomy OAG development in phakic and pseudophakic eyes.
Retrospective case series of 101 eyes of 101 patients who had vitrectomy for idiopathic epiretinal membrane or idiopathic macular hole with a follow-up duration of more than 6 months. Eligible patients were assessed for new development of OAG during the follow-up period. Patients were separated into two groups based on the lens status at the end of the vitrectomy for statistical analysis using Kaplan-Meier survival analysis with log-rank test and Cox regression analysis.
The mean follow-up duration was 51 months (range, 6-80 months). Of the 101 eyes, 8 (7.9%) eyes developed OAG during the follow-up period. Phakic eyes were less likely to develop OAG after vitrectomy compared with pseudophakic eyes, with 2% and 13%, respectively (log-rank test, P = 0.025). The result remained statistically significant after adjustment for age, refractive error and the use of gas tamponade (adjusted odds ratio = 0.09, P = 0.038).
The presence of the crystalline lens may be protective against the late development of OAG after vitrectomy.
评估玻璃体切除术后开角型青光眼（OAG）的晚期发生情况，并比较有晶状体眼和人工晶状体眼玻璃体切除术后OAG的发生率。
对101例因特发性视网膜前膜或特发性黄斑裂孔行玻璃体切除术且随访时间超过6个月的患者的101只眼进行回顾性病例系列研究。评估符合条件的患者在随访期间OAG的新发病例。根据玻璃体切除术后结束时的晶状体状态将患者分为两组，采用Kaplan-Meier生存分析和对数秩检验以及Cox回归分析进行统计分析。
平均随访时间为51个月（范围6 - 80个月）。101只眼中，8只眼（7.9%）在随访期间发生了OAG。有晶状体眼玻璃体切除术后发生OAG的可能性低于人工晶状体眼，分别为2%和13%（对数秩检验，P = 0.025）。在对年龄、屈光不正和气体填充的使用进行校正后，结果仍具有统计学意义（校正比值比 = 0.09，P = 0.038）。
晶状体的存在可能对玻璃体切除术后OAG的晚期发生具有保护作用。
