For over 8 years the authors have used the inverted V-Y incision of the quadriceps, a modification of the Coonse and Adams approach, in total knee arthroplasty (TKA) with serious reduction of flexion, ankylosis, or limited mobility of the patella because of previous operations. This procedure not only allows easy and complete exposure, but improves the range of flexion without incision-related complications. Detailed results of a consecutive series of 16 knee arthroplasties are presented.
