Abstract

BACKGROUND

The anterolateral thigh flap is commonly used for reconstruction of various soft-tissue defects. This article presents the authors' approach to one-stage reconstruction of composite soft-tissue defects using an anterolateral thigh flap with a vascularized fascia lata.

METHODS

This retrospective review analyzed 973 patients who had undergone anterolateral thigh flap reconstruction for various soft-tissue defects over the past 10 years. Various types of complicated defects in 36 patients were reconstructed with a composite anterolateral thigh flap combined with vascularized fascia lata. The fascia lata component of the flap was used for abdominal wall and fascial defect reconstruction in 12 patients, for lip-cheek defect reconstruction in 15 patients, for reconstruction of composite defects in extremities in nine patients, and for reconstruction of the perineum in one patient. Functional outcomes of donor sites were investigated by using a dynamometer.

RESULTS

All flaps except one survived. The overall flap survival rate was 97 percent. Patients achieved satisfactory results without major postoperative complications. The study revealed that vascularized fascia may mimic a fascial sheath but lacks the muscle-synchronized excursion properties. Apart from a mild deficiency in quadriceps femoris muscles contraction in the donor thighs, no difficulties in daily ambulation were reported by the patients.

CONCLUSIONS

The anterolateral thigh flap with vascularized fascia lata provides a reliable fascial component for single-stage reconstruction of complex soft-tissue defects.

背景

方法

回顾性分析了过去 10 年来 973 例接受股前外侧皮瓣修复各种软组织缺损的患者。36 例复杂缺损患者采用复合股前外侧皮瓣联合带血管蒂阔筋膜进行修复。12 例患者的阔筋膜瓣用于腹壁和筋膜缺损重建,15 例患者用于唇颊缺损重建,9 例患者用于四肢复合缺损重建,1 例患者用于会阴重建。使用测力计研究供区的功能结果。

结果

除 1 例外,所有皮瓣均存活。总的皮瓣存活率为 97%。患者术后无重大并发症,结果满意。研究表明,带血管蒂阔筋膜可以模拟筋膜鞘,但缺乏与肌肉同步的活动特性。除了供区大腿股四头肌收缩轻度不足外,患者在日常活动中没有报告任何困难。

结论
