PURPOSE
To report a fixed dilated pupil and anterior subcapsular cataract formation following Descemet stripping endothelial keratoplasty (DSAEK).
METHODS
A 47-year-old man with Fuchs endothelial dystrophy underwent DSAEK of the right eye. The preoperative slit lamp examination was normal. Mydriatic drops were used at the end of procedure.
RESULTS
Postoperatively, the pupil remained fixed and dilated with no right direct or consensual response to light, or constriction during accommodation throughout a 1-year follow-up period. Slit lamp examination after air resorption revealed a transient anterior subcapsular cataract and posterior synechiae. Based on these findings, a diagnosis of Urrets-Zavalia syndrome was made.
CONCLUSION
To our knowledge, this is the first report of Urrets-Zavalia syndrome after DSAEK.
目的
报告一例接受 Descemet stripping 内皮角膜移植术（DSAEK）后出现固定性瞳孔散大及前囊下白内障。

一名 47 岁男性，患有 Fuchs 内皮营养不良，右眼接受 DSAEK 手术。术前裂隙灯检查正常。手术结束时使用散瞳滴剂。
结果
术后，瞳孔持续固定散大，无光直接或间接对光反应，在 1 年的随访期间，调节时也无收缩。空气吸收后裂隙灯检查显示一过性前囊下白内障和后粘连。根据这些发现，诊断为 Urrets-Zavalia 综合征。
结论
据我们所知，这是首例 DSAEK 术后发生 Urrets-Zavalia 综合征的报告。
