Abstract

AIM

Spasm of the internal anal sphincter plays a role in haemorrhoidal disease and may be a source of anal pain after haemorrhoid surgery. In this study, we investigated the effect of topical diltiazem (DTZ) on postoperative pain following Milligan-Morgan haemorrhoidectomy.

METHOD

After haemorrhoidectomy, 33 patients were randomly assigned to receive DTZ ointment (2%; n = 16) or a placebo ointment (n = 17). Ointments were applied to the perianal region three times daily for 7 days. A Visual Analogue Scale was used to assess postoperative pain. The primary end-point was reduction in postoperative pain.

RESULTS

Patients using the DTZ cream had significantly less pain and greater benefit than those in the placebo group throughout the first postoperative week, and total and daily narcotic analgesic use was higher in the placebo group. There were no differences in morbidity between the two groups.

CONCLUSION

Perianal application of DTZ cream after haemorrhoidectomy significantly reduces postoperative pain and is perceived as beneficial, with no increase in associated morbidity.

目的

内肛门括约肌痉挛在痔病中起作用,可能是痔手术后肛门疼痛的一个来源。本研究旨在探讨肛门局部应用地尔硫䓬(DTZ)对痔环切术后疼痛的影响。

方法

痔切除术后,33 例患者被随机分为 DTZ 软膏(2%;n=16)或安慰剂软膏(n=17)组。软膏每天 3 次应用于肛周,持续 7 天。采用视觉模拟评分法(VAS)评估术后疼痛。主要终点是术后疼痛的减轻。

结果

使用 DTZ 乳膏的患者在整个术后第 1 周的疼痛明显减轻,获益明显大于安慰剂组,且安慰剂组的总用和日用麻醉性镇痛药用量更高。两组的发病率无差异。

结论

痔切除术后肛门局部应用 DTZ 乳膏可显著减轻术后疼痛,被认为是有益的,且不会增加相关发病率。
