In patients without a history of previous blood transfusions or pregnancy, alloimmunization to HLA-antigens can be virtually prevented by rigorous white cell depletion of blood components. In the present state of knowledge it is recommended that white cells should be depleted prior to storage of blood and that "bedside" filtration should be omitted. Patients who are presensitized by past blood transfusions or pregnancy are still at considerable risk of activation of the secondary antibody response by HLA-class I antigens still present in white cell depleted blood components.
对于既往无输血或妊娠史的患者,通过严格去除血液成分中的白细胞,几乎可以预防对HLA抗原的同种免疫。根据目前的知识水平,建议在血液储存前去除白细胞,并且应省略“床边”过滤。因既往输血或妊娠而致敏的患者,仍有相当大的风险因白细胞去除后的血液成分中仍存在的HLA - I类抗原而激活二次抗体反应。
