A 76-year-old man with a severe neck deformity resulting in a facedown posture presented with a visually significant cataract in his right eye. The patient had been scheduled for cataract surgery elsewhere, but the surgery was cancelled in the operating room when he could not be positioned beneath the operating microscope. The patient's surgery was safely and effectively performed by placing 25 to 30 pillows under his buttocks and legs and one pillow and a towel beneath his head and neck. The patient and pillows were taped to the operating table so that they would not move.
