To report a masquerade syndrome secondary to multiple myeloma iris infiltration.
Observational case report.
A 74-year-old Caucasian woman presenting with unilateral hypertensive uveitis and past medical history significant for multiple myeloma underwent aqueous cytology and a trabeculectomy with iridectomy.
Cytology revealed atypical plasma cells in the aqueous while the anatomopathologic analysis of iris tissue demonstrated a substitution of the iris tissue by neoplastic plasma cells. After surgery and a new cycle of chemotherapy best-corrected visual acuity and intraocular pressure improved.
Masquerade syndrome should always be considered in elderly uveitis. A correct diagnosis can be life-saving.

观察性病例报告。
一名 74 岁的白人女性，单侧高血压性葡萄膜炎，有既往多发性骨髓瘤病史，行房水细胞学检查和小梁切除术伴虹膜切除术。
细胞学检查显示房水中有异型浆细胞，而虹膜组织的解剖病理分析显示肿瘤浆细胞替代了虹膜组织。手术后和新的化疗周期后，最佳矫正视力和眼压均有所改善。
老年葡萄膜炎时应始终考虑伪装综合征。正确的诊断可以挽救生命。
