Clinical significance of circulating CD4(+) T cell subsets, including T-helper (Th)1, Th2, Th17 and regulatory T (Treg) cells, in patients with atopic dermatitis (AD) remains unclear. No previous studies have simultaneously evaluated the four T cell subset profiles in AD.
The aim of the present study was to explore whether the percentage of these four subsets of CD4(+) T cells correlate to the severity parameters of AD patients.
Intracellular expression of interferon (IFN)-γ, interleukin (IL)-4, IL-17 and forkhead box P3 (Foxp3) in CD4(+) T cells was evaluated in peripheral blood mononuclear cells from normal controls and patient with AD as well as with chronic eczema using a flow cytometer. Serum CCL17 levels were measured as an objective severity parameter of AD together with percentage of eosinophils and serum IgE levels.
In AD patients, the number of Th1 (IFN-γ(+)) and Th17 (IL-17(+)) subsets was significantly decreased, but that of Th2 (IL-4(+)) and Treg (Foxp3(+)) subsets was similar to that of normal controls. The T cell subset profiles of patients with chronic eczema were not different with those of normal controls. The frequency of Th17cells, particularly that of the IFN-γ(nega)IL-17(+) subset, showed a significant negative correlation with CCL17, IgE and eosinophil levels in AD patients. This was, however, not the case in Th1, Th2 and Treg cells.
Decreased circulating Th17 cells might contribute to activity of AD.
特应性皮炎(AD)患者循环 CD4+T 细胞亚群(包括辅助性 T 细胞 1(Th1)、Th2、Th17 和调节性 T(Treg)细胞)的临床意义尚不清楚。以前的研究尚未同时评估 AD 患者的这四种 T 细胞亚群谱。
本研究旨在探讨这四种 CD4+T 细胞亚群的比例是否与 AD 患者的严重程度参数相关。
采用流式细胞术检测外周血单个核细胞中干扰素(IFN)-γ、白细胞介素(IL)-4、IL-17 和叉头框 P3(Foxp3)在正常对照者、AD 患者和慢性湿疹患者 CD4+T 细胞中的表达情况。同时测定 CCL17 水平作为 AD 的客观严重程度参数,以及嗜酸性粒细胞百分比和血清 IgE 水平。
在 AD 患者中,Th1(IFN-γ(+))和 Th17(IL-17(+))亚群的数量显著减少,但 Th2(IL-4(+))和 Treg(Foxp3(+))亚群与正常对照组相似。慢性湿疹患者的 T 细胞亚群谱与正常对照组无差异。AD 患者 Th17 细胞的频率,尤其是 IFN-γ(neg)IL-17(+)亚群,与 CCL17、IgE 和嗜酸性粒细胞水平呈显著负相关。然而,Th1、Th2 和 Treg 细胞则并非如此。
循环 Th17 细胞减少可能有助于 AD 的活动。
