Effects of partial portal vein arterialization on the hilar bile duct in a rat model.

Abstract

BACKGROUND

Liver revascularization is frequently required during the enlarged radical operation for hilar cholangiocarcinoma involving the hepatic artery. Researchers have carried out a number of experiments applying partial portal vein arterialization (PVA) in clinical practice. In this study we aimed to establish a theoretical basis for clinical application of partial PVA and to investigate the effects of partial PVA on rat hilar bile duct and hepatic functions.

METHODS

Thirty rats were randomly and equally assigned into 3 groups: control (group A), hepatic artery ligation+bile duct recanalization (group B), and partial PVA+bile duct recanalization (group C). Proliferation and apoptosis of rat hilar bile duct epithelial cells, arteriolar counts of the peribiliary plexus (PBP) of the bile duct wall, changes in serum biochemistry, and pathologic changes in the bile duct were assessed 1 month after operation.

RESULTS

The proliferation of hilar bile duct epithelial cells in group B was greater than in groups A and C (P<0.01). No apoptotic hilar bile duct epithelial cells were detected in any of the groups. The PBP arteriolar counts of the hilar bile duct wall were similar in groups A and C (P>0.05), but the count was lower in group B than in group A (P<0.01). No statistically significant differences in alanine aminotransferase, aspartate aminotransferase, alkaline phosphatase and albumin were found in the 3 groups. The gamma-glutamyltransferase value was higher in group B than in groups A and C (P<0.01). The hepatic tissues of groups A and C showed no significant abnormality. Chronic inflammatory changes in the hilar bile duct walls were observed only in group B.

CONCLUSION

Partial PVA can restore the arterial blood supply of the hilar bile duct and significantly extenuate the injury to hilar bile duct epithelial cells resulting from hepatic artery ligation.

背景

在涉及肝动脉的肝门部胆管癌扩大根治术中,常需进行肝动脉重建。研究人员在临床实践中进行了多次部分门静脉动脉化(PVA)实验。本研究旨在为临床应用部分 PVA 提供理论依据,并探讨部分 PVA 对大鼠肝门胆管和肝功能的影响。

方法

30 只大鼠随机等分为 3 组:对照组(A 组)、肝动脉结扎+胆管再通组(B 组)和部分 PVA+胆管再通组(C 组)。术后 1 个月,评估各组大鼠肝门部胆管上皮细胞增殖和凋亡、胆管壁周围胆丛(PBP)小动脉计数、血清生化指标变化及胆管病理改变。

结果

B 组大鼠肝门部胆管上皮细胞增殖大于 A 组和 C 组(P<0.01),各组均未见凋亡的肝门部胆管上皮细胞。A 组和 C 组肝门部胆管壁 PBP 小动脉计数相似(P>0.05),但 B 组低于 A 组(P<0.01)。3 组丙氨酸氨基转移酶、天冬氨酸氨基转移酶、碱性磷酸酶和白蛋白差异均无统计学意义。B 组γ-谷氨酰转肽酶值高于 A 组和 C 组(P<0.01)。A 组和 C 组肝组织未见明显异常,仅 B 组肝门部胆管壁有慢性炎症改变。

结论

部分 PVA 可恢复肝门部胆管的动脉血供,明显减轻肝动脉结扎对肝门部胆管上皮细胞的损伤。
