CT and MRI findings in a patient with retrocardiac paraoesophageal omental herniation are presented and the literature reviewed. Characteristic CT features included a bilobed, retrocardiac fatty mass, with branching blood vessels throughout, a midline septum and extension through the diaphragm. MRI confirmed the fatty nature of the bilobed, intrathoracic mass and clearly defined pedicles of fat, from each lobe, extending into the abdomen through the oesophageal hiatus of the diaphragm.
