Valley Baptist Medical Center has dropped its 30-day readmission rate to 15% by improving communication with patients and family members as well as post-acute providers. After a pilot project for heart failure patients, the hospital expanded the project to all admissions on medical units. The team adapted the Project RED (Re-Engineering Discharge) model to improve the discharge process. The hospital participates in communitywide meetings with post-acute providers organized by the TMF Health Quality Institute to improve transitions in care.
