A critical evaluation of the "Cooperative By-Pass Study" is necessary for the management of ischaemic cerebro-vascular disease. On this basis further optimizing of the EC-IC-Bypass technique is essential. A new technique for microvascular anastomosis originally developed by von Tulleken et al., is presented as a method and with electron-microscope observations.
