Impacts of histological prostatitis on sexual function and lower urinary tract symptoms in patients with benign prostatic hyperplasia.

Abstract

OBJECTIVE

To investigate the correlation of histological prostatitis with sexual function (erectile dysfunction [ED]) and lower urinary tract symptoms (LUTS) in patients with benign prostatic hyperplasia (BPH).

METHODS

A retrospective analysis of patients with BPH who received surgical treatment (from May 1, 2012 to November 30, 2012) was conducted, consisting of 80 patients with uncomplicated BPH and 80 patients with BPH plus histological prostatitis. The International Index of Erectile Function (IIEF-5) symptom score and the International Prostate Symptom Score (IPSS) before surgery were calculated. Preoperative sexual functions were compared between the 2 groups.

RESULTS

Differences between both groups in age (72.56 ± 7.36 vs 71.98 ± 7.33) and IPSS score (18.65 ± 5.72 vs 20.50 ± 7.12) were not statistically significant (P >.05). Meanwhile, comparison in erectile function symptom score (14.80 ± 5.93 vs 7.35 ± 4.38) demonstrated significant differences (P <.001). According to the IIEF-5 score, 52 patients had normal erectile function or mild ED, 16 had moderate ED, and 12 had severe ED in the uncomplicated BPH group, whereas 10 patients had mild ED, 32 had moderate ED, 38 had severe ED, and no patients were found normal in BPH within the histological group. Further analysis using the chi-square test demonstrated significant differences between both groups (P <.001).

CONCLUSION

BPH combined with histological prostatitis had a serious impact on sexual function of the patients. Histological prostatitis may serve as a major risk factor for sexual dysfunction while having little effects on LUTS in patients with BPH.

方法

对 2012 年 5 月 1 日至 11 月 30 日接受手术治疗的 BPH 患者进行回顾性分析,包括 80 例单纯 BPH 患者和 80 例 BPH 合并组织学前列腺炎患者。计算术前国际勃起功能指数(IIEF-5)症状评分和国际前列腺症状评分(IPSS)。比较两组患者术前性功能。

结果

两组患者在年龄(72.56±7.36 岁比 71.98±7.33 岁)和 IPSS 评分(18.65±5.72 分比 20.50±7.12 分)方面差异无统计学意义(P>.05)。同时,勃起功能症状评分(14.80±5.93 分比 7.35±4.38 分)比较差异有统计学意义(P<.001)。在单纯 BPH 组中,52 例患者勃起功能正常或轻度 ED,16 例中度 ED,12 例重度 ED;而在 BPH 合并组织学组中,10 例轻度 ED,32 例中度 ED,38 例重度 ED,无患者勃起功能正常。进一步采用卡方检验分析显示两组差异有统计学意义(P<.001)。

结论

BPH 合并组织学前列腺炎对患者性功能有严重影响。组织学前列腺炎可能是 BPH 患者性功能障碍的主要危险因素,对 LUTS 影响较小。
