An angiographically verified case of aplasia of both vertebral arteries is described. Blood was supplied to the whole vertebro-basilar territory by the hypertrophied left occipital artery the direct continuation of which was the basilar artery of the brain. The article also deals with other anomalies of the vertebral arteries which are important in surgery.
