Bakers are known to experience respiratory complaints ranging from allergic rhinitis to chronic bronchitis and asthma. We report here the first confirmed case of baker's asthma in Singapore. The patient is a 40 year-old Chinese male, working at a biscuit factory for the last 14 years as a mixer. He developed asthmatic attacks five years after joining the factory and had been on bronchodilators and oral steroids. He was confirmed to have occupational asthma due to wheat flour, following a bronchial challenge test. He was subsequently transferred to another section and there was marked improvement in the frequency of attacks and the peak expiratory flow rate. This case illustrates the need to consider workplace factors in the diagnosis and management of asthma among the working population. Medical treatment alone is insufficient in the management of occupational and work-related asthma.
众所周知,面包师会出现从过敏性鼻炎到慢性支气管炎和哮喘等一系列呼吸道疾病。我们在此报告新加坡首例确诊的面包师哮喘病例。患者为一名40岁的中国男性,在一家饼干厂担任搅拌机操作员已有14年。他在入职该厂五年后出现哮喘发作,一直在使用支气管扩张剂和口服类固醇药物。经过支气管激发试验,他被确诊为因小麦粉导致的职业性哮喘。随后他被调至另一部门,哮喘发作频率和呼气峰值流速均有显著改善。该病例表明,在诊断和管理职业人群中的哮喘时,需要考虑工作场所因素。仅靠药物治疗不足以管理职业性和与工作相关的哮喘。
