The aim of our study was to evaluate the advantages and disadvantages of 3D plating system in the treatment of mandibular fractures.
20 mandibular fractures in 18 patients at various anatomic locations and were treated by open reduction and internal fixation using 3D plates. All patients were followed at regular intervals of 4(th), 8(th) and 12(th) weeks respectively. Patients were assessed post-operatively for lingual splay and occlusal stability. The incidence of neurosensory deficit, infection, masticatory difficulty, non-union, malunion was also assessed.
A significant reduction in lingual splay (72.2%) and occlusal stability (72.2%) was seen. The overall complication rate was (16.6%) which included two patients who developed post-operative paresthesia of lip, three patients had infection and two cases of masticatory difficulty which later subsided by higher antibiotics and 4 weeks of MMF. No evidence of non-union, malunion was noted.
A single 3D 2 mm miniplate with 2 mm × 8 mm screws is a reliable and an effective treatment modality for mandibular fracture.
本研究旨在评估三维接骨板系统治疗下颌骨骨折的优缺点。
18例患者共20处下颌骨骨折,骨折位于不同解剖部位,采用三维接骨板切开复位内固定治疗。所有患者分别在第4、8和12周定期随访。术后评估患者的舌侧张开和咬合稳定性。还评估了神经感觉功能障碍、感染、咀嚼困难、骨不连、骨畸形愈合的发生率。
舌侧张开(72.2%)和咬合稳定性(72.2%)显著降低。总体并发症发生率为(16.6%),其中包括2例患者术后出现唇部感觉异常,3例患者发生感染,2例咀嚼困难患者经加大抗生素用量及4周颌间固定后症状缓解。未发现骨不连、骨畸形愈合的证据。
单枚2毫米厚的三维微型接骨板及2毫米×8毫米的螺钉是治疗下颌骨骨折的一种可靠且有效的治疗方式。
