Quality of life (QOL) can be considered as a quality indicator of health care systems and nurses can play an important role to improve QOL in patients with congestive heart failure (CHF). The aim of this study was to determine the effects of education provided by nurses on the QOL in patients with CHF in home-visit care compared to usual care.
In a single-blind randomized controlled trial conducted from September 2011 to June 2012, 110 patients with CHF were randomly assigned into two groups. While patients in group I were received usual education at the time of hospital discharge, patients in Group II, in addition to usual education, were received special education regards to their illness by nurses who visited patients in their homes. The 36-item short-form (SF-36) questionnaire was used to evaluate the patient's QOL at the time of discharge and also six months after hospital discharge.
At the time of hospital discharge, mean score of all 8 sub-score of SF-36 questionnaire was 63.4±7.8 in patients of group II and 61.1±6.4 in patients of group I, respectively (P> 0.05). QOL was decreased in group I and increased in group II compared to the time of hospital discharge. After six months, mean score of QOL was higher in group II than in group I.
QOL of patients with CHF were decreased after hospital discharge. Education provided by nurses in home-visit care could improve the QOL in patients with CHF, based on the findings of this study.
生活质量(QOL)可被视为医疗保健系统的质量指标,护士在改善充血性心力衰竭(CHF)患者的生活质量方面可发挥重要作用。本研究的目的是确定与常规护理相比,护士提供的教育对居家护理的CHF患者生活质量的影响。
在2011年9月至2012年6月进行的一项单盲随机对照试验中,110例CHF患者被随机分为两组。第一组患者在出院时接受常规教育,而第二组患者除了接受常规教育外,还由上门访视患者的护士就其病情提供特殊教育。采用36项简明健康调查问卷(SF-36)在出院时和出院后6个月评估患者的生活质量。
出院时,SF-36问卷所有8个分项的平均得分在第二组患者中为63.4±7.8,在第一组患者中为61.1±6.4(P>0.05)。与出院时相比,第一组患者的生活质量下降,第二组患者的生活质量上升。6个月后,第二组患者的生活质量平均得分高于第一组。
CHF患者出院后的生活质量下降。基于本研究的结果,护士在居家护理中提供的教育可改善CHF患者的生活质量。
