We describe a case of a symptomatic os Vesalianum, a rarely found accessory ossicle of the foot, which should be considered as a possible source of lateral foot pain. The condition should be differentiated from the fifth metatarsal epiphysis and the os peronei in patients who have the roentgenographic finding of an ossicle immediately posterolateral to the base of the fifth metatarsal.
