Uterine anomalies are uncommon and in our environment they are usually incidental findings. There are different types of uterine anomalies and most are known to influence reproductive outcomes of the affected women. We present a case of a 26-year old G6P5+0 (3 alive) with a bicornuate uterus who presented with 12 weeks history of amenorrhoea, lower abdominal pain and bleeding per vaginam. Pelvic ultrasound showed an empty uterus and a right adnexal mass which was misdiagnosed as an ectopic pregnancy. At surgery, she was found to have a bicornuate uterus with an intact pregnancy. The pregnancy aborted spontaneously shortly after discharge from hospital. The patient had had five previous successful pregnancies which she carried to term with three of her children alive. The objective of the report was to show that patients with uterine anomaly could have successful pregnancy outcome; the congenital anomaly could be discovered incidentally while being managed for other clinical conditions.
