The evaluation of clinical competence in medical residents is a complex procedure. Teachers need reliable and valid instruments to evaluate objectively the, clinical competence. The aim of this study was to determine the reliability and validity of an instrument designed to evaluate the clinical competence of medical residents.
We designed an instrument taking into consideration every part of the clinical method, and three different levels of competence were determined for each one. The instrument was examined with regards to the clarity, pertinence and sufficiency of each clinical indicator by five expert pediatricians. The instrument was finally constituted by 11 indicators. Each resident was evaluated independently by three pediatricians.
A total of 651 measurements were done in 234 residents. The instrument distinguished between extreme groups, had a value of Cronbach's alpha of 0.778 and the factorial analysis set apart two factors: clinical competence and complementary competences. No statistical differences were found between evaluators neither in global evaluation or in each indicator.
The instrument here proposed is valid and reliable. It may be used in formative evaluation of medical residents in clinical specialization programs.
评估住院医师的临床能力是一个复杂的过程。教师需要可靠且有效的工具来客观评估临床能力。本研究的目的是确定一种旨在评估住院医师临床能力的工具的可靠性和有效性。
我们设计了一种考虑临床方法各个部分的工具,并为每个部分确定了三种不同的能力水平。五位儿科专家对该工具的每个临床指标的清晰度、相关性和充分性进行了检查。该工具最终由11项指标组成。每位住院医师由三位儿科医生独立评估。
共对234名住院医师进行了651次评估。该工具能够区分极端组,克朗巴哈系数为0.778,因子分析分离出两个因素:临床能力和补充能力。评估者之间在整体评估或每个指标上均未发现统计学差异。
本文提出的工具是有效且可靠的。它可用于临床专科项目中住院医师的形成性评估。
