To evaluate the influence of periodontal therapy on glycosylated hemoglobin and fasting blood glucose and serum levels of interleukin (IL)-4, IL-6, IL-8, IL-10, and tumor necrosis factor-alpha (TNF-α) in chronic periodontitis (CP) patients with type-2 diabetes mellitus (T2DM) and in controls.
A total of 30 periodontal patients, 15 of which were systemically healthy (control group), and 15 were T2DM patients (test group) were included in this study. This prospective study was carried out at Istanbul University, Istanbul, Turkey between February 2011 and December 2013. Plaque index, gingival index, bleeding on probing, periodontal probing depth, and clinical attachment level were assessed and recorded at baseline, one, and 3 months after therapy. Serum samples were collected at the same time-points and analyzed using Luminex assay for the levels of IL-4, IL-6, IL-8, IL-10, and TNF-α. The change in the metabolic control was also monitored.
All clinical parameters were significantly improved after the periodontal therapy in both groups (p less than l0.001). Glycosylated hemoglobin levels were decreased; however, the difference was not significant (p more than 0.05). Fasting blood glucose levels were decreased one month after therapy, and increased at 3 months. Patients with T2DM had significantly higher levels of circulating IL-8 at each time point, and TNF-α (p less than 0.05) at baseline. The IL-4 and IL-10 levels were decreased at one month after therapy (p more than 0.05).
Periodontal therapy has limited impact on the serum levels of IL-4, IL-6, IL-8, IL-10, and TNF-α. Metabolic control levels were not influenced by periodontal therapy.
评估牙周治疗对2型糖尿病(T2DM)慢性牙周炎(CP)患者及对照组糖化血红蛋白、空腹血糖以及白细胞介素(IL)-4、IL-6、IL-8、IL-10和肿瘤坏死因子-α(TNF-α)血清水平的影响。
本研究共纳入30例牙周病患者,其中15例全身健康(对照组),15例为T2DM患者(试验组)。这项前瞻性研究于2011年2月至2013年12月在土耳其伊斯坦布尔的伊斯坦布尔大学开展。在基线、治疗后1个月和3个月时评估并记录菌斑指数、牙龈指数、探诊出血、牙周探诊深度和临床附着水平。在相同时间点采集血清样本,采用Luminex分析法分析IL-4、IL-6、IL-8、IL-10和TNF-α的水平。同时监测代谢控制情况的变化。
两组患者牙周治疗后所有临床参数均有显著改善(p<0.001)。糖化血红蛋白水平降低,但差异无统计学意义(p>0.05)。治疗1个月后空腹血糖水平降低,3个月时升高。T2DM患者在各时间点循环IL-8水平均显著更高,基线时TNF-α水平也显著更高(p<0.05)。治疗1个月后IL-4和IL-10水平降低(p>0.05)。
牙周治疗对IL-4、IL-6、IL-8、IL-10和TNF-α的血清水平影响有限。牙周治疗未影响代谢控制水平。
