The significance of extended radical operation (gastric resection with R3 and paraaortic lymph-node mean value of R4 dissection) was retrospectively studied in 1,363 cases (1968-1987) of advanced gastric cancer in Stage IV. For liver metastasis, gastric and R2 lymph node resection with transcatheter arterial embolization or chemoembolization was superior to liver resection. The operative results showed that 5-year survival rate of radical dissection of RO-2 was 19.3% and that of R3-4 was 34.6% (p less than 0.05). It also suggested that the operation was effective in 78% of patients with POHO and POHO, ow(-) and aw(-). As a conclusion, extended radical operation is significant as the treatment of advanced gastric cancer in Stage IV.
对1968年至1987年间1363例IV期进展期胃癌患者进行回顾性研究,以探讨扩大根治术(R3胃切除术及平均R4主动脉旁淋巴结清扫)的意义。对于肝转移,胃及R2淋巴结切除联合经导管动脉栓塞或化疗栓塞优于肝切除。手术结果显示,RO-2根治性清扫的5年生存率为19.3%,R3-4为34.6%(p<0.05)。这也表明该手术对78%的POHO、POHO、ow(-)和aw(-)患者有效。总之,扩大根治术作为IV期进展期胃癌的治疗方法具有重要意义。
