Diagnosis of the pathophysiological mechanisms responsible for producing acute and chronic ischemia of the hand in patients with systemic disease provides a rational basis for treatment. The indications and techniques to restore or improve blood flow to the ischemic hand by microsurgical revascularization and digital sympathectomy are discussed.
