Abstract

OBJECTIVE

To evaluate the efficacy and safety of topical 5-aminolevulinic acid (ALA) mediated photodynamic therapy (PDT) for the treatment of severe acne vulgaris.

PATIENTS AND METHODS

A total of 125 patients with Pillsbury III-IV severe facial acne were treated with 3 courses of ALA-PDT with an interval of 2 weeks. ALA gel (3.6%) was applied topically to acne lesions for 1.5 h. The lesions were irradiated by a LED light of 633 nm with a light intensity of 66 mW/cm² and a light dose of 126 J/cm². Patients were evaluated for efficacy and safety at weeks 2, 4, 6, 8, 12 after the initial treatment.

RESULTS

The total effective rates were 1.6%, 24.8%, 68.8%, 89.6% and 88.8% at the 2- 4- 6- 8- and 12-week after the initial treatment respectively. The clinical outcomes were the best at 4 weeks after the final treatment. The best effective rate and cure rate were 89.6% and 44% respectively. 26 patients and 16 patients showed apparent exacerbation of acne lesions before the 2nd and 3rd treatment respectively, but all of them showed good or excellent improvement after 3-course treatment. Adverse reactions were mild and transient.

CONCLUSIONS

3.6% ALA-PDT is a simple, safe and effective therapeutic option for the treatment of severe acne vulgaris.

目的

评估外用5-氨基酮戊酸(ALA)介导的光动力疗法(PDT)治疗重度寻常痤疮的疗效和安全性。

患者与方法

总共125例患有Pillsbury III-IV级重度面部痤疮的患者接受了3个疗程的ALA-PDT治疗,疗程间隔为2周。将3.6%的ALA凝胶局部涂抹于痤疮皮损处1.5小时。皮损用波长633nm的LED光照射,光强度为66mW/cm²,光剂量为126J/cm²。在初始治疗后的第2、4、6、8、12周对患者进行疗效和安全性评估。

结果

初始治疗后第2、4、6、8、12周的总有效率分别为1.6%、24.8%、68.8%、89.6%和88.8%。最终治疗后4周临床疗效最佳。最佳有效率和治愈率分别为89.6%和44%。分别有26例和16例患者在第2次和第3次治疗前痤疮皮损明显加重,但在3个疗程治疗后均显示良好或极佳的改善。不良反应轻微且短暂。

结论

3.6%的ALA-PDT是治疗重度寻常痤疮的一种简单、安全且有效的治疗选择。
