To describe a patient with central retinal artery occlusion following an episode of mechanical strangulation.
A retrospective case report. An 83-year-old man presented with acute vision loss in his right eye after a strangling episode. Multimodal clinical imaging was performed.
Visual acuity in the affected right eye was no light perception. Examination revealed optic nerve edema and optic nerve pallor and also severe vascular attenuation and a cherry-red spot in the macula.
Mechanical disruption of carotid plaque can lead to occlusion of the central retinal artery, which may portend a dismal visual prognosis.
