PURPOSE
Neonates are likely to experience numerous painful procedures in neonatal intensive care units (NICUs). Parents have expressed a wish to be more involved in their infants' pain alleviation. The purpose of this study was to describe parents' perceptions concerning the factors that influence parental participation in pain alleviation in an NICU.
DESIGN AND METHODS
The qualitative study was conducted in level II and III NICUs (7 units) of Finland's four university hospitals. Data were collected through open-ended questionnaires and analyzed using inductive content analysis.
RESULTS
Factors that promoted parental participation consisted of five main categories: parental counseling by staff, parents' awareness of their own role, parents' motivation to participate in pain relief, family-friendly facilities and good communication. Factors hindering parental participation consisted of eight categories, including restrictive environment, lack of knowledge, everyday life requirements, underestimation of parents, the nature of the medical procedures, procedure- and pain-related emotions, deteriorated health status of the child and mother and (8) uncertainty of parenting.
CONCLUSIONS
This study revealed a number of factors that are important to take into account when improving parental involvement in neonatal pain alleviation. Especially, parental participation can be promoted by providing sufficient counseling based on the parents' needs and creating facilities that support parents' participation.
PRACTICE IMPLICATION
Parents should be engaged as partners in caregiving and decision making, and they should be given space to assume the role of parents during their child's hospitalization.
目的
新生儿在新生儿重症监护病房(NICU)可能会经历许多痛苦的治疗程序。父母表示希望更多地参与到减轻婴儿痛苦的过程中。本研究的目的是描述父母对于影响其在NICU中参与减轻婴儿痛苦的因素的看法。
设计与方法
这项定性研究在芬兰四所大学医院的二级和三级NICU(共7个科室)进行。通过开放式问卷收集数据,并采用归纳性内容分析法进行分析。
结果
促进父母参与的因素主要包括五个类别:医护人员对父母的指导、父母对自身角色的认知、父母参与减轻痛苦的动机、家庭友好型设施以及良好的沟通。阻碍父母参与的因素包括八个类别,如环境限制、知识缺乏、日常生活需求、对父母的低估、医疗程序的性质、与程序和疼痛相关的情绪、孩子和母亲健康状况的恶化以及育儿的不确定性。
结论
本研究揭示了在提高父母对新生儿疼痛缓解的参与度时需要考虑的一些重要因素。特别是,可以通过根据父母的需求提供充分的指导以及创建支持父母参与的设施来促进父母的参与。
实践意义
应让父母作为护理和决策的合作伙伴参与进来,并在孩子住院期间给予他们承担父母角色的空间。
