The authors describe a rare case of pancreatopleural fistula in a 42-year-old patient as a complication of a post-traumatic pseudocyst of the pancreas resistant to repeated conservative treatment. After establishment of a pancreatojejunal anastomosis side-to-side with an excluded jejunal loop and ligature of the fistula, regression of the pleural effusion and healing was recorded.
